To be considered, we need three things:

1. A completed form. You can submit the form online below, or click here if you would like to print and mail the form.

2. High-res images. You can mail them on a disc or send them via a file-sharing service to Editor Kara Hagerman, khagerman@cincinnatimagazine.com.

3. A signed release from your photographer. The printable form above (the link in item No. 1) includes a photography release form.
